Few clarifications:

no cancel button on dashboard when adding a widget

Cancel button is straight below widget list

got a crash report when I tried to create a mysql database

Were you using "MySQL" section or "Websites > Database" section (Ajenti V beta)?

after plugin installation - have to restart or relogon for plugin list to refresh?

Ajenti restart is required.
